How Do I Know If I Need A Minimal Hair Transplant?

When it comes to determining what qualifies as a minimal hair transplant, it is helpful to refer to the Norwood scale. The Norwood Scale was created as an easy to reference guide with imagery that that can help a doctor determine which stage of hair loss a patient is at, compared to what is typical for each stage of balding. This can also be helpful for determining approximately how many follicular grafts will be necessary for a successful hair transplant. A minimal hair transplant would require approximately 1000 follicular grafts or less.

Norwood Scale Stages 1-2 

The Norwood Scale is broken into seven stages. Each stage describes the pattern of hair loss and its severity. Stage one displays no significantly visible hair loss or recession of the hairline. At this stage, it’s unnecessary to have a hair transplant. At Stage 2 there is a slight recession of the hairline, and the hair might be thinning at the temples. This is sometimes known as a mature, or adult hairline. That’s because as we age, a certain amount of hair thinning is normal. Some patients will wish to undergo a transplant at this stage, especially if they are at a hair class 2a where the center front of the hairline displays more obvious thinning.

As a general estimate, it will take between 500-800 follicular grafts for patients on the 2 or 2a of the Norwood Scale. Getting a transplant at this stage is a great way to increase the fullness of the hair around the face, improving the symmetry of the hairline. This is also the ideal stage for getting a minimal hair transplant. As we go up on the Norwood scale a patient will most likely require more than 1,000 grafts. 

Norwood Scale Stages 3-4 

For example, a patient at Stage 3 on the Norwood Scale displays the first signs of clinically significant hair loss. By this stage, the hairline is deeply recessed from the temples. As a result, the hair will have a pattern that resembles the shape of either an M, U, or V. The receded areas will either be completely bald or have very sparse hair with a lot of scalp visible.

A Hamilton-Norwood Scale & hair class of 3a will also include significant hair loss at the vertex or top of the scalp. A patient with a Norwood 3 will normally require anywhere from 1,000-1,400 follicular grafts. At this stage, the hair at the back of the head is evaluated to determine that there are enough adequate grafts to harvest for transplanting at the back of the head.

When it comes to a Norwood scale of 4 or higher it will be necessary to transplant more than 1000 grafts, and would no longer be considered a minimal hair transplant.

It will take as long as a year for your hair transplants to grow out after surgery. In the first month or so following surgery, the newly transplanted hair will fall out before your new hair grows in. This is completely normal and a sign that your transplant was a success. Many patients choose to incorporate Minoxidol or low-level laser therapy with laser caps to help encourage the growth of their transplants. This is not necessary but it is certainly something you can discuss with your doctor as far as your long-term treatment plan.
