Even with the newer Follicular Unit Transplant (FUT), if not done correctly the hair transplant result can be catastrophic. The FUT method harvests “strips” of hair-bearing tissue from the donor area. These strips are then dissected into smaller, individual grafts for implantation. The issue that can arise from this method is that the strips that are harvested can cause excessive scarring in the donor area if not harvested correctly. The scar normally manifests in a strip across the donor area and if not properly cared for will lengthen and widen, becoming more pronounced and apparent. 

Scalp reduction surgery has been a hair restoration practice since the 1970s. This is a procedure where a surgeon will make a series of incisions across the balding area of the scalp, then pull and stretch the hair-bearing scalp over the removed, bald scalp and finally suture it in place. Scalp reduction surgery is an outdated hair restoration procedure due to the risk of infection, suture retraction, bleeding and a condition called “stretch back.” Stretch back is when the stretched part of the hair-bearing scalp that has been stitched together loses its tightness and stretches out partially or totally, leaving a visible bald area created by the stretching. This dog ear scar is not only unattractive; it is extremely difficult to repair.

What is a Free Flap?

Another terrifying antique hair restoration procedure is called the Flap and Free Flap procedure. This method is when the doctor will slice a flap of your scalp out of your head and reposition it onto the balding area of your head. The regular flap method is when they will slice the flap on just three sides and then twist it around so it’s facing the opposite direction. This allows the blood circulation to continue in the tissue, (sometimes) but leaves an unsightly “knot” where the tissue was twisted.

The Free-form Flap method is when they slice the flap on all four sides and just stitch it back into where they want it on your scalp. Some of the side effects of this method include necrosis, infection, and loosened skin in the forehead that develops and hangs over the brow, giving a Frankenstein-like or Neanderthal appearance.  It goes without saying that the Flap method of hair transplants was not terribly popular for very long. Nowadays, it’s only generally used for burn victims and those suffering from disfigurement.
