What is FUE and How is it Different?

It’s true that the FUE hair transplant takes a great deal of more time to complete than the FUT process. The extraction process in a FUT transplant involves the surgeon removing a “strip” of hair-bearing tissue from the specified donor area of the scalp. After strip removal, doctors suture the scalp back together. Then the surgeon would dissect the individual follicles from this strip, with each follicle containing 1 to 4 hairs.

With the FUE extraction process, instead of removing a whole strip of tissue so the scalp had to be sutured back together, the doctor will extract each hair follicle individually with a special hole punch tool. This tool is basically a hollow scalpel with a diameter of 0.7 to 1.0 millimeters. It is placed over the hair follicle and pressed down and rotated to separate the tissue of the scalp from the hair follicle so that it can be removed with forceps. Over the years there have been many improvements in the tools used for the extraction phase of FUE, but more on that later.

The implantation of the hair follicles is similar to both the FUT, and FUE hair transplant surgery. After the hair follicle extractions, the doctor or surgeon makes a small incision in the recipient area of the scalp and carefully implants the hair follicle. This implantation must be carefully done so that the implanted follicle is in a line and angle of the naturally growing hair. If the implantation is placed poorly, when the hair follicle begins to grow, it will do so at an angle different than the naturally-growing hair on the scalp. Consequently, this creates an unnatural appearance.

Because the FUE surgery does not require sutures, it involves much less post-procedure pain and discomfort. Although it does take longer to complete, being able to individually extract hair follicles allows the surgeon much more control over the extraction phase. Also, the FUT procedure leaves a long linear scar along the donor area of the scalp, which is usually along the rear, base of the skull. This scar is readily apparent if the hair on the back of the head is short, as men’s haircuts often will be. 

The scarring from the FUE method leaves small, pinprick-like scars where the follicles extractions occur. Even with short hair, these scars are hard to spot. For this reason, the FUE method of hair transplants has become much more popular over the years than the FUT technique. However, there are instances where FUT could be called for.

There are still some hair experts that prefer the regular, manual punch tool for FUE transplants. The SAFE (Surgically Advanced Follicular Extraction) system is a tool that uses a sharp, circular punch to incise the skin around the hair follicle, then a blunt circular punch that loosens the follicle from the surrounding tissue. This tool minimizes human error in the extraction phases of an FUE procedure. If the surgeon damages the root, or “bulb” of the hair follicle during extraction, then the follicle will die after implantation, and no hair will grow from it. Doctors called this a transection. The SAFE tool minimizes transections and speeds up extractions in the FUE procedure.

The Powered SAFE Scribe

This FUE instrument uses the technology of the SAFE system and motorizes it. The Powered SAFE Scribe has a rotating sharp punch and blunt tip that allows both the incising of the skin around the follicle and the removal of the follicle to happen simultaneously. The proponents of the Powered SAFE Scribe claim that it drastically speeds up the process of follicle extraction and minimizes the transactions that occur.

RotoCore

The RotoCore is a handheld FUE instrument that automatically rotates while incising the scalp around the hair follicle. The RotoCore also controls the depth at which it will incise the skin. This is said to prevent transections as well as prevent instances where a surgeon will core too deep into the skin of the scalp. This tool is supposed to decrease hand fatigue on the part of the surgeon as well as reduce scarring during the extraction phase. 

NeoGraft

NeoGraft is a mechanical device that operates similar to the Powered SAFE Scribe but uses suction to extract the follicle after the skin has been incised instead of a blunt tool. The NeoGraft hair device will then automatically move the follicle to a tray to await implantation. The NeoGraft device has been marketed to doctors and dermatologists as an “All-in-One” FUE Hair Transplant device.

ARTAS System

The ARTAS System is the most mechanized FUE device. ARTAS is a robotic arm that is equipped with a Powered SAFE Scribe tool that is operated by a computer and run by computer programming.
